If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.

If you are already registered with us and have moved outside of our catchment area please fill in the out of area application form.
Once we have received this, your application will be passed to the GP's to decide whether they feel you can stay registered here or it would be better for you to re-register locally.
Please note out of area patients are not entitled to home visits, health visiting or district nurse services. If you will be requiring these services you will need to re-register at a local GP surgery.
