The service is provided to patients over 55 years of age and struggling with hearing. Patients can use any of these providers by calling them directly.

Community Nursing Team
A community nursing team may include district nurses, community nurses, healthcare support workers, community matrons, rapid response nurses, wound clinic nurses, and end of life care nurses.

Community Occupational Therapy and Physiotherapy
Our community therapy services are provided in the home and other community settings for vulnerable, elderly or frail people and their carers. Our community therapy services work in partnership with Derbyshire Adult Social Services and other statutory and voluntary agencies.
We provide physiotherapy and occupational therapy assessment, treatment and advice for individuals in their own home, working jointly with patients and their carers to establish goals and priorities.

Continence Service
We are a team of bladder and bowel specialist nurses with expertise in the care and treatment of bladder, bowel and pelvic health conditions, including pelvic floor dysfunction, continence and incontinence support.
